Abstract:

Objetivo: Comparar la eficacia diagnóstica entre auto - toma de muestras y la toma hecha por el ginecólogo para la prueba del VPH . Sujetos y Métodos: Se incluyeron 120 pacientes elegidas del cribado de rutina de cáncer de cuello uterino, y 120 pacientes diagnosticadas en los últimos 6 meses de LSIL. Obtuvimos consentimiento por escrito de todas las pacientes. Las muestras para la detección del HPV fueron obtenidas por un ginecólogo o con el dispositivo de autotoma, y enviadas para la prueba de VPH molecular al departamento de Anatomía Patológica en el Hospital Universitario Son Espases (Mallorca, España. Resultados: En el grupo de la auto- toma se consiguió una sensibilidad del 0.76 y una especificidad del 0.83. En el grupo de muestra cervical, obtuvimos la misma sensibilidad de 0.76 y especificidad similar de 0.85. Conclusiones: La concordancia entre las dos técnicas utilizando el índice Kappa fue muy buena : 0,86 ( IC del 95 % , 0,79 a 0,93). Palabras clave: Cáncer de cérvix, Auto-toma, HPV, cribado ; Objective: To compare the diagnostic efficacy for HPV testing between samples obtained with the Selfcitotest self-sampling device and samples obtained by gynecologists with the cervical sampler kit. Pacients and Study design: We included 120 patients who had come for routine screening of cervical cancer and 120 patients with a previous cytology with diagnostic of Low grade Squamous Intraepithelial Lesion (LSIL) .We obtained written consent of all patients. Samples for molecular HPV testing were obtained by gynecologists or by self-sampling and were sent to the Pathology Department in University Hospital Son Espases (Mallorca, Spain). Results: The self-sampling group achieved a sensitivity of 0.76 and a specificity of 0.83. In the cervical sampler group we obtained the same sensitivity of 0.76 and similar specificity of 0.85. Conclusions: The agreement between the two techniques using the Kappa index was very good: 0.86 (CI 95%, 0.79 to 0.93). Keywords: Cervical cancer, Self-sampling, HPV, screening |
